Abstract

Introduction

Medication errors are a critical challenge in modern healthcare systems, often resulting in serious patient harm. Clinical pharmacists are uniquely positioned to prevent these errors by ensuring appropriate prescribing, dispensing, and administration. This article focuses on the impact of clinical pharmacist interventions in reducing medication errors in Indian hospitals.

Clinical Pharmacist Interventions and Error Reduction

Clinical pharmacists detect and correct errors during prescription validation, patient counseling, and drug monitoring. A North Indian hospital study reported a 38% drop in prescription-related errors with pharmacist involvement [1]. Their role during ward rounds improves prescribing practices and ensures timely therapeutic recommendations [2].

They also conduct educational workshops and simulations that enhance clinical staff awareness of drug safety practices. In emergency departments, pharmacists play a pivotal role in preventing dose miscalculations and contraindicated drug use [3]. Their review of high-alert medications and drug interactions significantly reduces adverse events [4].

Use of electronic prescription audits combined with pharmacist oversight enhances safety and ensures adherence to treatment guidelines [5].
